The health risk assessment does not replace seeing your own doctor. In fact, in order to get full benefit of this assessment, you should know your current height and weight, blood pressure, cholesterol (including HDL), and glucose (blood sugar). If you do not have this information, we still encourage you to take the assessment. For more information on these health checks, contact your local healthcare provider or Four Corners Health Department for assistance. When you click on the link below you will find yourself at the first login page. This will ask for a login ID and a password. To make this easy, we’ve included the proper login which is public and the first password which is health. Once you have typed in these words then click Login. This will take you to a second page where you will be asked to create a unique username and password. This is important so that you can log back into the system to access your personal health assessment survey in the future or print your summary report. Please choose a username and password that you’ll be able to remember. An easy-to-remember username would be first initial and last name or first name and last initial (minimum of 6 characters).
